Day visits -12 week block - 2008

Activity booked for group of up to eight participants for twelve consecutive 2 hour session.  programme designed by qe2activitycentre staff in conjunction with visiting staff and service users.

GROUP SIZE: up to eight participants
LENGTH OF SESSION: two hours
SESSION TIMES: 

 

morning:          10:00 - 12:00

afternoon:         1:00 -   3:00

late afternoon    3:00 -   5:00
COST:

June 30th - September 19th,

September 22nd - December 12th

Block booking - £600.00 for 12 week block (£50 per session)
PAYMENT: is due before the start of the session. You will be invoiced in advance
CANCELLATIONS:

cancellations are charged at full fees.  The Centre, reserves the right to cancel a booking for whatever reason.  In this case all monies paid to the Centre in respect of the booking will be refunded in full

CONSENT FORMS: consent forms must be completed and signed for each participant
NOTE: 

Service users must be accompanied by an appropriate level of staffing. 

QE II SJ Activities Centre staff are responsible for the safe running of activity sessions.

Visiting staff retain overall responsibility for control of client’s behaviour and have responsibility for health issues.

 Visitors are responsible for the cost of repairs made necessary by any damage they may cause
